  • Describe Cardiometabolic disparities in the U.S. and the southeastern U.S. (Medical Knowledge)
  • Identify benefits of using Community Health Worker models to reach low-income and minoritized populations. (Patient Care)
  • Recognize models for sustaining CHW programs for chronic disease prevention & management. (Systems-based Practice)
