Identify indicators that a neonate is ready for palliative care transition from the NICU to home, hospice, or another hospital unit.
Discuss the roles and responsibilities of the interdisciplinary team in coordinating seamless palliative care transitions.
Describe the essential components of a comprehensive transition plan, including symptom management, equipment needs, and caregiver education
Demonstrate effective communication strategies for discussing care transitions with families.
Recognize potential barriers to successful transition and formulate strategies to address them (e.g., insurance issues, home care availability, emotional readiness).
