Pharmacy in Family Medicine: A Contemporary Interprofessional Model of Practice
This seminar is intended to generate a robust discussion among Medical and Pharmacy colleagues on developing or enhancing existing innovative primary care practice models engaging pharmacists to serve patients cared for by MCW clinicians. The role of pharmacists is evolving from dispensing to providing patient centered care. Evidence is available that show the value of pharmacists in health care system models. Adding pharmacists to primary care teams have been shown to improve patient outcomes, decrease readmissions rates and improve overall savings.
