Program Description: 

This CME program addresses gaps in medical knowledge and best practices with regards to the clinical practice of internal medicine and teaching learners the discipline of internal medicine.  These conferences update the audience with new findings and best practices to match current understanding in the field of internal medicine so that our faculty are knowledgeable, competent, and practice patient care of value to the patient.

1. Understand the different ways that High Need High Cost patients are categorized and how readmission risk is identified

2. Name 4 evidence based methods of reducing the frequency of high need high cost readmissions and effective programs that utilize these methods

3. Identify how Froedtert and the Medical College of Wisconsin are addressing high risk transitions through the Hospital to Home Care Team
