This one-day program is designed to provide primary care physicians with education about cancer survivorship and foster opportunities for care collaboration. This forum will identify strategies for best care of this patient population and lay a foundation toward a dedicated team.

Learning Objectives
Participants who engage in this educational intervention will be able to:
- to identify key late adverse effects of cancer treatments, to better prepare them to provide appropriate surveillance and preventive care.
- become more familiar with monitoring for cancer recurrence.
- will gain the latest breast cancer screening guidelines and familiarity with Froedtert High Risk Breast Cancer Program.
