This presentation will illustrate recent findings in the understanding of the pathogenic mechanisms underlying thrombotic thrombocytopenic purpura (TTP). Furthermore, this talk will focus on how to make diagnosis of congenital and acquired TTP. Prof. Peyvandi will cover traditional and the most recent advanced therapies. 

Marie Scully "Management of pregnancy in patients with TTP"

This presentation will provide an understanding of the impact of TTP presenting in pregnancy, differentiation from other TMAs, treatment in subsequent pregnancies and increased recognition of congenital TTP precipitated by pregnancy.

Biographies

Flora Peyvandi

Flora Peyvandi is an Associate Professor of Internal Medicine at the University of Milan and the Director of the Angelo Bianchi Bonomi Haemophilia and Thrombosis Centre, Fondazione IRCCS Ca’ Granda, Ospedale Maggiore Policlinico, Milan, Italy. Her entire academic and clinical career has been dedicated to the knowledge of the underlying causes of coagulation diseases for the purpose of determining the best medical approach to treatment for patients. Testifying to this from the scientific point of view are more than 240 studies published in well-reputed specialized journals as well as the positions she holds in World Federation of Hemophilia and ISTH. As Chair of the Factor VIII, Factor IX, and Rare Coagulation Disorders ISTH-SSC subcommittee she helped to develop a great number of projects that focused on harmonizing clinical methods used in laboratories as well as evaluating the effectiveness and safety of therapeutic products.
Since 1998, she has been the recipient of more than 40 project grants funded by Italian and International organisations. In particular she has been the principal investigator of a project on “Establishment of an European Network of Rare Bleeding Disorders” funded by the commission of the European Communities - Public Health Executive Agency and she is now recipient as leader of the workpackage dedicated to the prospective study of rare bleeding disorders within the project “European Haemophilia Network (EUHANET)” funded by the commission of the European Union - the Executive Agency for Health and Consumers. In 2014 she was awarded the “Great Hippocrates," as the Italian medical researcher of the year.

Marie Scully

Dr Marie Scully is a consultant haematologist at UCLH, London, UK with an interest in haemostasis, thrombosis and blood transfusion, where she is clinical lead. She has a particular interest in platelet mediated disorders, specifically, ITP, TTP and aHUS. Primary publications include TTP, in particular treatment and clinical subtypes, in conjunction with an understanding of the pathogenesis of the underlying disease. Grants with the Medical Research unit, the British Heart Foundation and Baxter Global and has unrestricted educational grants from Octapharma and Alexion. She supervises postgraduate doctorate and involved in undergraduate and postgraduate teaching and regularly reviews for haematology and related medical journals.

Target audience: 

This presentation is intended to provide an overview of new insights of TTP knowledge to a specialized audience of hematologists and internal medicine physicians interested in the care of TTP. Due to the variability of presenting symptoms and of the associated conditions, this talk may be also intended for a variety of physicians, such as neurologists and nephrologists, which may observe initially cases of TTP.

Learning objectives: 

At the end of this Webinar, the participants will be able to:

  • Describe advances in understanding the pathogenic mechanisms and the following clinical implications in TTP
  • Discuss what tests need to be done for congenital and acquired TTP diagnosis
  • Illustrate an overview of the literature with the results of clinical trials and/or observational studies on novel targeted therapies and to show the novel therapies on TTP
  • Discuss potential predictive markers of relapse and implications on treatment management of patients during remission
  • TTP and aHUS can be precipitated by pregnancy. They may present in any gestation, from the first, through to post-partum.
  • Late onset congenital TTP may present as an initial manifestation in pregnancy
  • Improved maternal and fetal outcomes in subsequent pregnancies in women with congenital TTP are associated with regular plasma infusion.
  • Women with a history of acquired TTP, can proceed with further pregnancies, associated with monitoring. Normal ADAMTS 13 activity levels at the onset of pregnancy are associated with improved maternal and fetal outcomes.
  • Monitoring of patients with TTP is required into the postpartum period given the increased risk of presentation at this time.
