Perioperative Management of Patients Who Are Receiving Anticoagulant
Based on findings from a recent educational needs assessment (unpublished), clinicians’ main challenges are in the areas of diagnosis and testing and treatment of thrombotic and bleeding disorders.
This activity is targeted to the management of patients who are receiving anticoagulant treatments but require an emergency surgery or procedure. The activity will help clinicians learn how patients’ treatment can be managed to mitigate the risk of bleeding based on which therapies they are receiving and how emergent the procedure is.
The number of available treatments has proliferated in recent years with many new anticoagulant therapies becoming available leading to a complex landscape of treatment options and reversal procedures. This educational activity will provide learners with the most up to date information on how to manage bleeding risk for patients receiving a range of different therapies.
Target Audience
Physicians treating patients who are receiving anticoagulant therapy who require surgery.
Learning Objectives
- Identify procedure and minor surgery types in which patients' anticoagulants can be safely continued, without interruption.
- Manage the perioperative interruption and resumption of VKAs in patients with either atrial fibrillation (AF), a mechanical heart valve (MHV) or venous thromboembolism (VTE) who require an elective surgery/procedure.
- Identify VKA-treated patients who need heparin bridging during VKA interruption and to be able to administer heparin bridging in a safe manner.
- Manage the perioperative interruption and resumption of DOACs in patients who require an elective surgery/procedure.
- Identify when patients need perioperative coagulation testing.
- Manage patients on VKAs or DOACs who need an urgent surgery/procedure.
