School-Based Asthma Management Program (SAMPRO)

The goal of this QI Project is to increase the creation and delivery of Asthma Management Plans and School Forms to school age asthmatics in southeastern Wisconsin, by at least 20% in 3 months of implementation, using educational programs and standardized asthma plans. 

 

The first step is to watch a 17 minute educational video on the School Based Asthma Management Program and asthma management plans. 

You will then download the MOC workbook and complete 2 PDSA cycles. 

When you have completed 2 PDSA cycles and the MOC workbook, email the workbook to Liz Heimerl-Rolland, MOC Manager (eheimerl@mcw.edu)

 

The Medical College of Wisconsin designates this QI Activity for a maximum of 30 PI-CME Points and 20 MOC points for the American Board of Family Medicine and 30 Practice Assessment MOC Points for the American Board of Internal Medicine.

Maintenance of Certification (MOC) Part IV - ABMS

To obtain MOC Part IV credit, the practitioner is required to maintain an "active participation role" meaning the practitioner must:

  • Provide direct or consultative care to patients as part of the QI project.
  • Implement the project’s interventions (the changes designed to improve care) and reflect on the tools selected to see if they were the correct tools to make a change.
  • Collect, submit and review data in the project’s measurement plan with two linked cycles of data review.
  • Collaborate with others (at least two people) through active discussion of your learning.
