Participants who engage in this educational intervention will be able to:
- Understand the epidemiology and barriers that young adults with chronic conditions of childhood origin facing transitioning to adult care.
- Develop a framework to measure outcomes of transfer to adult care.
- Learn models from two academic medical centers to address transfer from pediatric to adult medical care.
