There is a growing awareness there will be an ongoing shortage of hospice and palliative medicine clinicians and a need to improve generalists' hospice and palliative care skills in order to meet the large volume of unmet palliative care needs among seriously ill Americans. Major medical groups including the ACGME, USMLE, AMA, LCME, and the Robert Woods Johnson Foundation Federation have recognized the need to develop effective hospice and palliative care curricula to meet this knowledge gap. As a result of these effort, end of life care education has become mandated in US medical schools, there has been an organized effort to include hospice and palliative care content in textbooks and specialty board exams, and faculty development projects are availa ble nationally to improve hospice and palliative care skills. While these efforts have helped, there have been recognized shortfalls. At the root of the problem with regards to hospice and palliative care education is the "confidence-competency gap" - clinicians often have an over-inflated sense of confidence regarding their skills and knowledge in this field, which erodes the educational effectiveness of these more traditional learning formats by leading to a low level of educational tension .

Many hospice and palliative care educational experts have highlighted the need for "just in time" educational and clinical resources which can be accessed at the time of clinical need in hospice and palliative care to help overcome the confidence-competency gap.

Fast Facts and Concepts are a collection of over 300 one-paged, evidenced-based and peer-reviewed clinical educational tools designed to answer a targeted clinical issue in hospice and palliative medicine. They capitalize on teachable moments.

Learning Objectives

Objectives:

Describe diagnostic and treatment approaches to 8 common opioid adverse effects
Differentiate two different types of reactions commonly described as opioids allergies.
Describe a comprehensive treatment approach to the prevention and management of opioid constipation.
Describe the indications, contraindications, and expected outcomes for nine interventional analgesic therapies for patients with serious illnesses.
List two different approaches to the use of radiation therapy as an analgesic modality.
Describe clinical situations when spinal analgesic approaches should be considered.
Define addiction, opioid abuse, involuntary diversion and voluntary diversion and be able to distinguish these terms from physical dependence and opioid tolerance.
Describe at least two evidence-based screening tools for identifying opioid misuse and abuse in seriously ill patients.
Safely manage the symptoms of opioid withdrawal when opioids are discontinued.
Describe starting doses for two oral and two parenteral opioids in palliative care.
List three opioid equi-analgesic and dose escalation principles.
Describe three principles of writing opioid prescription orders in the inpatient and outpatient settings.
