
Building a Seamless GDMT Approach Across Care Settings - October 19, 2026
Join us for an interactive one-hour lunch-and-learn focused on optimizing the care of patients with heart failure across the continuum of care. This educational program will review current guideline-directed medical therapy (GDMT), discuss practical strategies for initiating and titrating evidence-based therapies, and explore opportunities to improve transitions of care through interdisciplinary collaboration. Participants will gain practical, case-based insights that can readily be applied in clinical practice to enhance patient outcomes.
Target Audience
This educational activity is designed for physicians, pharmacists, physician assistants, nurse practitioners, and other healthcare professionals involved in the care of patients with heart failure. The program is intended for clinicians practicing in both inpatient and outpatient settings who are interested in expanding their knowledge of GDMT, improving transitions of care, and increasing their confidence in the evidence-based management of heart failure.
