From Discharge to Destination Program
Join us for an interactive one-hour continuing education program focused on optimizing the care of patients with heart failure across the continuum of care. This educational program will review current guideline directed medical therapy (GDMT), discuss practical strategies for initiating and titrating evidence-based therapies, and explore opportunities to improve transitions of care through interdisciplinary collaboration. Participants will gain practical, case-based insights that can readily be applied in clinical practice to enhance patient outcomes.
Target Audience
This educational activity is designed for invited physicians, pharmacists, physician assistants, nurse practitioners, and other healthcare professionals involved in the care of patients with heart failure. The program is intended for clinicians practicing in both inpatient and outpatient settings who are interested in expanding their knowledge of GDMT, improving transitions of care, and increasing their confidence in the evidence-based management of heart failure.
Learning Objectives
- Interpret current heart failure guideline recommendations, including guideline-directed medical therapy (GDMT) to support evidence-based clinical decision-making across care settings
- Apply evidence-based strategies for the initiation, titration, and optimization of heart failure therapies to improve patient outcomes and reduce the risk of hospitalizations, including readmissions
- Develop multidisciplinary transitions-of-care approaches that promote continuity of heart failure management across inpatient, outpatient, and primary care settings
- Examine common misconceptions and perceived contraindications that contribute to the underutilization of GDMT and formulate evidence-based strategies to address these barriers
