Together In Care is a year-long webinar based educational series focused on neonatal-perinatal palliative care concepts for neonatal teams. Our goals include equipping neonatal team members with the competencies and comfort to utilize primary palliative care skills and recognize when to collaborate with specialty palliative care.

Participants who engage in this educational intervention will be able to:
1. Identify indicators that a neonate is ready for palliative care transition from the NICU to home, hospice, or another hospital unit.
2. Discuss the roles and responsibilities of the interdisciplinary team in coordinating seamless palliative care transitions.
3. Describe the essential components of a comprehensive transition plan, including symptom management, equipment needs, and caregiver education.
4. Demonstrate effective communication strategies for discussing care transitions with families.
5. Recognize potential barriers to successful transition and formulate strategies to address them (e.g., insurance issues, home care availability, emotional readiness).
